is there a form for request form bid so that i could bid on the house for painting. Does any one have bid request form or idea can someone help me i need it so badly. Project manager keeps asking me to bring bid request for what i charge for the things need to be painted. he also asked me to bring worker compensation and liablity insurance copya and fill out w-9 forms..

I need help from you guys....so badly...some one help me....

Oh I forgot to say: never give out your basic rates not even to your competition. Also when I am asked about rates off the cuff....
I simply say I will need to see the job, or see the plans to provide a fair price to you.
